Replacing a Lost CDL in West Virginia

You can apply for a replacement commercial driver's license with the West Virginia Division of Motor Vehicles (DMV) in person or POSSIBLY online (see below). Your steps will vary depending on whether you have a “not for federal" or “for federal" (REAL ID) license.

DO NOT operate your commercial truck or any commercial vehicle until you have a replacement CDL in your possession. Consider reporting your lost CDL to local and state authorities to help protect yourself against identity theft.

How to Get a Duplicate CDL in WV

In Person

You can order a replacement CDL in person at your local WV DMV office. Your requirements will vary slightly depending on whether you want a “not for federal" or “for federal" (REAL ID) license.

Not for Federal Identification

If you're replacing a license that isn't valid for federal identification purposes (i.e. does not have the gold star), head to your local DMV office with:

  • A completed Application for Commercial Driver's License (CDL) and/or Endorsements (Form DMV-CDL-1).
  • You can complete this form at home or request one at the office.
  • 1 document proving identity.
  • 2 documents proving WV residency.
    • You must provide 2 documents proving residency if your address has changed since your last CDL.
  • Document(s) providing your legal name change if your name has changed since your last CDL.
    • Depending on your situation, you might need to provide specific or multiple proofs of your name change. The WV DMV recommends calling the main DMV Call Center at (800) 642-9066 for instructions related to your specific situation.
  • Medical Examiner’s Certicate.
  • The $7.50 fee.

Generally, the WV DMV issues your duplicate CDL before you leave; however, your DMV agent will advise you on the process if it must mail the card.

For Federal Identification (REAL ID)

If you're replacing a license that is valid for federal identification purposes (i.e. has the gold star), your process will be similar to that of applying for a non-federal card except you must meet a few extra requirements.

Visit your local DMV office with:

  • A completed Application for Commercial Driver's License (CDL) and/or Endorsements (Form DMV-CDL-1).
    • Complete the application at home or obtain one at the office.
    • Mark that you want a “For Federal Identification" card.
  • 1 document proving identity.
  • 2 documents proving WV residency.
    • These must include your name and physical address.
  • 1 document proving your Social Security number.
  • Proof(s) of your legal name change if your name has changed since your last CDL.
    • The WV DMV recommends calling the main DMV Call Center at (800) 642-9066 for a list of which proofs(s) you must bring depending on your situation (some situations require multiple documents).
  • Medical Examiner’s Certicate.
  • All applicable fees.
    • $7.50 fee for your replacement CDL.
    • $10 fee for the REAL ID distinction.

Because you're getting a REAL ID, you'll receive a temporary commercial driver's license at the office and the DMV will mail your permanent duplicate to the address on file.

Online

Some drivers may be eligible to apply online for duplicate licenses; conveniently, the WV DMV's online application system checks your eligibility for you. Be prepared to provide personally identifying information so the system can find you.

If you're eligible, the online ordering process will walk you through the steps. Once you complete the application, you'll receive a duplicate license request confirmation and should receive your replacement CDL by mail.

Out-of-State WV CDL Holders

If you're currently out of state, you MIGHT be able to apply for a replacement CDL online; the state's online application system will check your eligibility.

The WV DMV's other application option is in person; however, because you're out of state, that's not convenient for you. Contact the DMV with details about your specific situation and find out if there are other application options available for you.

Meanwhile, DO NOT operate your commercial vehicle until you have your replacement CDL in your hand.
