DLD & DMV Forms in Utah

The Utah Department of Public Safety's (DPS) Driver License Division (DLD) and the UT Division of Motor Vehicles (DMV) provide several online driver and vehicle forms, respectively. You can download the online forms below as long as you have Adobe's free PDF Reader.

Keep in mind, some forms may only be available in person at your local DLD or DMV agency. Otherwise, you may find what you're looking for on our Utah motorcycle manual page or driver handbook page.

DLD6a: Application For License; Utah Driver License Division application for a regular, motorcycle, OR commercial permit OR license. Can also be used to apply for a UT identification card.

SS-5: Application for a Social Security Card; Order a new, replacement, OR corrected Social Security card.

TC-656: Vehicle Application for Utah Title; Apply for an original, replacement, transferred, OR branded Utah title with this Division of Motor Vehicles form.

TC-817: Application for Personalized Plates and Replacement of Existing Personalized Plates; Order new OR replacement personalized Utah license plates from the Division of Motor Vehicles.

TC-123: Application for Utah Duplicate Title; Request a duplicate UT vehicle title from the DMV.

TC-162: Application for Utah Motor Vehicle Identification Number; Utah DMV application for a new vehicle identification number (VIN) on a car that's been reconstructed or that's missing its original VIN.

TC-661: Certificate Of Inspection; Vehicle/Hull identification number (VIN/HIN) verification form for cars and vessels being registered in Utah for the first time.

TC-810: Exemption of Utah Safety and Emission Requirements for Vehicles Not in Utah; Claim exemption from Utah's required safety AND/OR emissions inspection(s) if your vehicle meets the qualifications outlined on this DMV form.

TC-569A: Ownership Statement; Form used to verify ownership of a vehicle if the Utah title is missing or has incorrect information.

TC-656V: Vessel Application For Utah Title; Request a new, transferred, OR corrected Utah vessel title from the DMV. Can also be used to change lienholder information on your vessel title.

TC-843: Bill of Sale; Use the Division of Motor Vehicles' bill of sale as a proof of purchase after transferring ownership of a vehicle in UT.

TC-353: Notice to Buyer of Vehicle Initially Delivered for Sale Country Other Than US; Provide this DMV form to a vehicle buyer if the car was originally bought/sold in a foreign country.

TC-891: Odometer Disclosure Statement; Use this Division of Motor Vehicles form to disclose the mileage on a car you're selling in Utah.

Ticket & Violations Forms

Rev. 2016: Notice of Appeal from Justice Court; Request to appeal a traffic ticket/violation verdict through the Utah Justice Court.

Commercial Forms

CDL-42: Commercial Driver License Medical Self Certification; If you're applying for a Utah CDL, use this DLD form to self certify as an intrastate OR interstate excepted OR non-excepted commercial driver.

649-F: Medical Examination Report for Commercial Driver Fitness Determination; Submit this driver fitness form to the DLD when applying for a UT commercial driver's license. Form must be completed by a physician.

MCSA-5876: Medical Examiner's Certificate; Certain Utah CDL holders are required to complete and carry this medical examiner's certificate.

Military Forms

CDL-ST WVR: Application for Military Skills Test Waiver; Request a CDL skills test waiver if you recently drove a commercial vehicle during your service in the military. Must be signed by your commanding officer.

Disability Forms

TC-842: Disabled Person And Physician Disability Certification; Apply for Utah disabled license plates OR an original or replacement window placard. The DMV requires a doctor to complete this form.
