Most Requested Forms

MV-1001: Application for Motor Vehicle Title & Registration; Title and register your vehicle with the South Dakota Division of Motor Vehicles.

SS-5: Application for a Social Security Card; Apply for an original, replacement, OR corrected Social Security card.

Form 1025: Bill of Sale; Use the DMV's bill of sale form as a proof of purchase when transferring ownership of a vehicle in SD.

Title & Registration Forms

MV-102: Affidavit for Duplicate License Plate/Validation Sticker; Request replacement license plates AND/OR registration decals from the SD Division of Motor Vehicles.

MV215: Affidavit of Vehicle Ownership by Succession; Submit this affidavit to the DMV to transfer ownership of a vehicle into your name after the owner has passed away if you meet the criteria listed on the form. MUST be notarized.

1006: Application For Replacement of License Plates, Validation Stickers or Lost Title Document (Lost in Mail); Request replacement registration stickers and license plates OR a duplicate vehicle title from the DMV if the originals were lost in the mail after you made an initial application.

MV-010: Application for a Duplicate Certificate of Title; Request a duplicate South Dakota vehicle OR boat title by submitting this form to the DMV. MUST be notarized.

MV-1001: Boat Title and Registration Application; Title and register your boat in South Dakota.

2040: Exemptions Form; Claim exemption from South Dakota's excise tax for registering vehicles if you meet the criteria listed on the form.

1015: Four Wheel, All Terrain Vehicle Affidavit; Submit this affidavit to the DMV when registering a four-wheel ATV, verifying you've installed all the required equipment.

Buying & Selling Forms

MV2200: Seller's Report of Sale; Submit this form to the DMV after selling a vehicle in South Dakota to release yourself from liability for the vehicle.

Form 1021: Uniform Mileage Disclosure Statement; Use this form to disclose the number of miles on a vehicle you're selling in South Dakota.

Commercial Forms

649-F: Medical Examination Report for Commercial Driver Fitness Determination; All SD commercial driver's license applicants must undergo a medical exam and turn this form in to the DMV. Must be completed by a medical professional.

MCSA-5876: Medical Examiners Certificate; Certain CDL applicants need to carry this medical certificate, which must be completed by a doctor.

REV 2014: Medical Self Certification Statement; When applying for a South Dakota CDL, use this form to self-certify as an excepted OR non-excepted interstate OR intrastate commercial driver.

Military Forms

CDL-SK TST WVR: Application for Military Skills Test Waiver; Request to have the CDL skills test waived if you recently drove a commercial vehicle in the military. Must be completed by your commanding officer.

DMV-1302: Application for Veteran License Plates and Affidavit; Apply for new, renewed, OR reassigned South Dakota veteran license plates with the Division of Motor Vehicles. MUST be notarized.

1303: Military License Plate Application; Apply for military license plates with the South Dakota DMV.

1305: Application for Physically Disabled Parking Permit and License Plates; Request original, replacement, renewal, permanent, OR temporary disabled parking permits OR license plates from the South Dakota DMV.

76: Voter Registration and Absentee Ballot Request Federal Post Card Application; If you're a U.S. citizen currently living outside the country, use this form to register to vote AND/OR request an absentee voting ballot.
