Replacing a Rhode Island Driver's License

If your Rhode Island driver's license is lost, stolen, or damaged, you must request a replacement license from the Rhode Island Division of Motor Vehicles (DMV).

This page will help you with the documents to bring and steps to take in order to obtain your duplicate license.

Apply for a Duplicate Driver's License

The steps you'll need to take to replace your Rhode Island driver's license are the same whether your license is lost, stolen, or damaged.

You must apply in person at your local Rhode Island DMV location. At this time, you CANNOT request a replacement state ID or driver's license online.

NOTE: If you are temporarily outside of Rhode Island, please read our section "Out-of-State Replacement Requests" below for more information.

In Person

To replace a lost, stolen, or damaged driver's license, visit your local Rhode Island DMV office and bring the following:

  • Application for License, Identification Card and Permits (Form LI-1).
  • 1 document to prove your identity, such as your:
    • U.S. birth certificate.
    • U.S. passport.
  • 1 document that shows your Social Security Number, such as your:
    • Social Security card.
    • W-2 or 1099
  • 2 documents that prove your RI residency.
  • Payment for the $27.50 duplicate license fee.

Out-of-State Driver's License Replacement

You can request a replacement Rhode Island driver's license or ID card by mail if you meet any of the following criteria:

  • Seasonally out-of-state.
  • Temporarily out of the state due to school, work, or other travel.
  • Outside of Rhode Island on U.S. military assignment.

To get started with your request for a replacement driver's license, you must complete and sign the Application for License, Identification Card and Permits (Form LI-1).

Be sure to include a check or money order for the $27.50 duplicate RI driver's license fee.

Contact the Rhode Island DMV at (401) 462-5862 if you do not receive a hard copy of your driver's license in the mail within 60 days.

Reporting a Missing License in Rhode Island

You are not required to report a lost or stolen driver's license in Rhode Island. However, if you are concerned that your driver's license was stolen or could be used to commit fraud or identity theft, you may want to consider filing a police report and notifying the RI DMV offices directly.

At the same time, you can contact the Rhode Island DMV to notify them and begin the process of getting a replacement driver's license. You can also ask for your DMV file to be flagged in case someone else tries to use your information to get a new driver's license or ID card fraudulently.
