DL-143: Non-Commercial Drivers License Application For Renewal; Submit this form when you need to renew your driver's license with the PA Department of Transportation.

DL-180: Non-Commercial Learners Permit Application; Application for a non-commercial PA learner's permit.

DL-180C: Parent or Guardian Certification Form; Form used to verify a minor in your care has completed the required number of hours of driving practice while holding a Pennsylvania learner's permit.

REV-336: Application for Pennsylvania Boat Registration Renewal; Application for boat registration AND/OR title.

MV-426B: Application for Reconstruct, Specially Constructed, Collectable, Modified, Flood, Theft Vehicles, Street Rods; Application to title a vehicle classified as reconstructed, collectable, modified, recovered from theft or flood OR street rod in Pennsylvania. MV-3: Motor Vehicle Verification of Fair Market Value by the Issuing Agent; Submit this form to PennDOT to verify the fair market value of a car you're selling if that vehicle is older and/or has depreciated significantly in value.

MCSA-5876: Medical Examiners Certificate; Submit this medical examination form as part of your PA commercial driver's license application. Must be completed by a medical professional.

DL-143CD: Renewal of a Commercial Drivers License; Submit this form to the PA Department of Transportation to renew your CDL.

DL-11CD: Self-Certification Form; Used to disclose if you'll be an interstate OR intrastate commercial driver. Required when applying for a Pennsylvania CDL.
