Driving Records in Louisiana

You can order your own driving record, or the driving record of another Louisiana driver, from the Department of Public Safety and Corrections (DPS) Office of Motor Vehicles (OMV).

Your driving record, also called an official driving record (ODR), contains information about your driving history, traffic violations, and driver license status. It can be used:

  • To adjust your car insurance policy rates.
  • In legal proceedings.
  • To verify personal information for pre-employment screenings and background checks.

The Louisiana DMV offers an official driving record (ODR).

Your driving record contains:

  • Traffic accident statements.
  • Your driver license status, which includes:
    • Suspensions.
    • Revocations.
    • Cancellations.
  • Your commercial driver's license (CDL) status, if applicable.
  • Traffic violations and convictions.

Violations and convictions can stay on your driving record from 3 years to 10 years, depending on the severity of the violation. Accidents and traffic violations can lead to fines, a suspended driver license and increased insurance rates.

How to Order Your Own Louisiana Driving Record

You can order your Louisiana official driving record (ODR) online, by mail, or in person.

How to Get Louisiana Driving Record Online

Official driving records requested online can be viewed on your computer or printed within 30 days after your transaction.

To order your driving record online, visit the Louisiana DMV website. You'll need:

  • Your name, address, date of birth, license class, and driver license number.
  • A printer to print your driving record.
  • Your mailing address.
  • A credit/debit card for your payment. (See “Fees for LA Driving Records" below.)

NOTE: You cannot use the online driving record request system between 11:30 p.m. and 4:00 a.m.

By-Mail & In-Person Driving Record Requests

To request your official driving record (ODR) by mail or in person:

  • Complete the Louisiana ODR Request Form (Form R0800).
  • Include a money order or check with your driving record fee. (See “Louisiana Driving Record Fees" below.)
  • Mail your application and payment to:

    OR
  • Submit your request and payment in person at one of the following Louisiana DMV offices:
    • Alexandria.
    • Baton Rouge.
    • Lake Charles.

Order Another Person's Driving Record

You can order the driving record of another Louisiana driver if you have authorized use under the Federal Driver Privacy Protection Act, including:

  • Government agencies.
  • Employers and businesses conducting background checks.
  • Insurance companies investigating claims and adjusting policy rates.

You can also request someone else's record with written permission from the individual on the record.

By-Mail & In-Person Requests

To request another driver's official driving record (ODR) by mail or in person:

  • Complete the LA ODR Request Form (Form R0800). Include:
    • The driver's information and authorization:
      • Name.
      • Driver license number.
      • Date of birth.
      • Signature to authorize the release of information to you.
    • Your name, signature and driver license number.
  • Include a money order or check for the driving record fee. (See “Louisiana Driving Record Fees" below.)
  • Mail your application and payment to:

What are the Louisiana Driving Record Fees?

The fee for an ODR depends on how you order it:

  • Online: $16 plus a $2 electronic commerce fee.
  • By mail or in person: $16.

Accepted forms of payment also differ by ordering method:

  • Online: Credit/debit card.
  • By mail: Check or money order.
  • In person at an Louisiana DMV office:
    • Cash.
    • Check or money order.

Correct Errors on Your Driving Record

If there is an error on your driving record, you should contact the Louisiana DPS Office of Motor Vehicles (OMV) at (225) 925-6146. Be sure to describe the exact nature of the error, and be prepared to provide proof in order to have it corrected expediently.
