Top Registration Sticker
To apply for a duplicate sticker, either call (608) 266-1466 anytime and leave a message, or send an e-mail to bur-veh-services.dmv@dot.state.wi.us. No matter the method, include the following information:
- Your full name, including the middle initial
- Your full mailing address, with zip code
- Your license plate number
- The type of sticker you're requesting
Also include your phone number or e-mail address where you can be reached during business hours, just in case more information is needed to process the request.
In most cases, your replacement sticker will be sent the next business day at no charge.
Top Registration
Just go to a Division of Motor Vehicles (DMV) service center with photo ID to replace your registration card. The duplicate card costs $2, or $4 for Fast Service.
Top License Plates
Non-Personalized Plate Replacement
If you have standard issue or specialty plates that have not been personalized, begin the replacement process by completing the replacement license plate form. Report stolen plates to your local police department.
Standard plates cost $2 each to replace. "University of Wisconsin" and "Celebrate Children" plates cost $6 each to replace, while all other specialty plates cost $5 a piece.
If you conduct the transaction at a DMV office, you'll also pay a $5 service fee. But, you'll get your plates on the spot. Just make sure the office handles plate transactions. And, also bring your title.
To replace your plates through the mail, send the completed replacement form and a check for the applicable fee made payable to the Registration Fee Trust to:
- Wisconsin Department of Transportation
- P.O. Box 7949
- Madison, WI 53707-7949
You'll receive a registration card with your new plate number within three to four weeks. The plates will follow two to four weeks later.
Be sure to destroy any remaining plates once you've received the replacement plates.
Personalized Plate Replacement
Those with personalized specialty plates or standard plates should also complete the replacement license plate form. Be sure to report stolen plates to your local police department.

For stolen personalized plates, it's a good idea to request a different personalization for your new plates. If you don't, the police may pull you over for driving with stolen plates.
Replacement personalized plates are mailed within six to eight weeks. Destroy any remaining plates once you've received the replacement plates.
