    Top Reporting an Incompetent Driver

Contact the Wisconsin Department of Transportation's (WisDOT) Medical Review Unit if you harbor suspicions that a driver is not medically fit to be operating a vehicle. To file a report, complete and submit a Driver Condition or Behavior Report form. You can snare this form at any DMV service center.

Positive driver identification is required for WisDOT to initiate action. If the driver is unknown to you, another person must verify the information contained in your report.

WisDOT will not investigate any driver based exclusively on old age or a diagnosis.
