    Top Driver License Division Forms

    Unlike the Utah DMV, the Utah Department of Public Safety's Driver License Division does not make most of its forms available online, so you will need to visit your local driver license field office to pick up forms (or you can fill them out there when you arrive).

    Forms you might need include a driver license application, a commercial driver license application, a driver license renewal, a change of name or address (or you can change your address online), a driving privilege card, a practice permit application, and an identification (ID) card.

    There are two forms that you can access online from the Driver License Division: the driving record request form and the request for accident report records.
