DMV Forms

When it comes to the Utah Division of Motor Vehicles (DMV), there's a form for practically everything. Fortunately, most DMV forms are available online, so you can print them out at home and fill them out on your own time before heading down to the DMV office. Forms for most vehicle-related activities can be viewed, downloaded, or printed from the main DMV forms page, including the following forms:

  • Registering your vehicle
  • Duplicate title
  • Application for Utah vehicle identification number (VIN)
  • License plates
  • Personalized or replacement license plates
  • Disabled person and physician disability certification (for disabled plates or placards)

Driver License Division Forms

Unlike the Utah DMV, the Utah Department of Public Safety's Driver License Division does not make most of its forms available online, so you will need to visit your local driver license field office to pick up forms (or you can fill them out there when you arrive).

Forms you might need include a driver license application, a commercial driver license application, a driver license renewal (you can also renew online), a change of name or address (or you can change your address online), a driving privilege card, a practice permit application, and an identification (ID) card.

There are two forms that you can access online from the Driver License Division: the driving record request form and the request for accident report records.
