Replacing a Lost License in Rhode Island

The upset of losing your driver's license is quickly lessened by the ease with which Rhode Island's Division of Motor Vehicles (DMV) lets you obtain a duplicate.
To request a replacement for a lost or stolen driver's license, report to any DMV location. You'll need to present one state-approved identity document and one state-approved signature document.
Once all of your information is confirmed, you'll then be asked to pay a $26.50 processing fee to receive your duplicate license.
Travelers
If you're traveling out of the state, in this country or out, and you lose your license, you can mail a notarized affidavit to the RI DMV requesting a duplicate. In the affidavit, you'll need to explain your reasons for being out of state and why you need a replacement.
Mail the notarized affidavit, a self-addressed stamped envelope (include international postage if you're out of the country), and a $26.50 fee to:
- Pawtucket DMV
- 100 Main St.
- Pawtucket, RI 02860
The DMV will mail a duplicate license to you.
