When your name changes, you need to change your Pennsylvania driver's license or ID card. Complete the DL-80 form and submit it, with the required documentation to:
- PA Department of Transportation
- Bureau of Driver Licensing
- P.O. Box 68272
- Harrisburg, PA 17106-8272
You'll need to submit a certified copy of your marriage certificate, divorce decree, or other court order showing your name change.
If, however, you want to use a name that differs with your birth name, spouse's surname, or name granted through a court order, you will need to provide a copy of an updated Social Security card, reflecting your name change, as well as two other sources (showing your new name) such as a tax record, a passport, a Voter Registration card, or any photo identification issued by a government agency.
An updated card will be issued in your new name. There is no cost.
To change your name with the Social Security Administration (SSA), visit the agency's site. Give the agency at least 48 hours to process your request.
Registrations & Titles
To change your name on your vehicle registration and title, complete Form MV-41, Application for Correction or Name Change. Complete side A if you only want to change the name within the state's records, and do not require a new title. There is no fee for this option.
If you want to get a hard copy of the title with changes made, complete Side B of the form and submit the other required documents and a title fee of $22.50. A new title and registration card will be mailed to you.

- Harrisburg, PA 17104-2516
You'll need to submit a copy of your current title, or pay an additional $22.50. You must also enclose a certified copy of your marriage certificate, divorce decree, or other court order showing your name change.
