Forms in Oregon
Most of the forms and publications for driver licenses, commercial driving, dealers and other Oregon Department of Motor Vehicles (DMV) requirements and matters are available online or by request for free. To request forms by mail, contact the DMV.
The DMV also provides a list of commonly requested records and their fees. There is another Web page for businesses or employers to order DMV records.
Among the most frequently downloaded forms are:
- Address Change, Notice of Voter Registration, fillable form or change address online
- Notice of Sale or Transfer of a Vehicle or give notice through an online form
These PDF forms require version 3.0 or better of Adobe Reader to be viewed or printed. Some forms are also "fillable," offered in FDF, allowing users to fill out forms on their computer screens, print them, and turn them in at a DMV office.
