Most of the forms and publications for driver licenses, commercial driving, dealers and other Oregon Department of Motor Vehicles (DMV) requirements and matters are available online or by request for free. To request forms by mail, contact the DMV.

The DMV also provides a list of commonly requested records and their fees. There is another Web page for businesses or employers to order DMV records.

• Bill of Sale for vehicles

These PDF forms require version 3.0 or better of Adobe Reader to be viewed or printed. Some forms are also "fillable," offered in FDF, allowing users to fill out forms on their computer screens, print them, and turn them in at a DMV office.
