    Oklahoma does not have one centralized agency for all things vehicular. Unfortunately, this makes it a bit of a challenge to get all of your licensing, registration, and other automotive needs met in once place; it also makes it impossible to obtain every form you need online.

    There are three main agencies you will need to know about for driver's licensing, license renewal, automobile registration, and driver's testing. Each one has its own form and facilities. Most of the websites list addresses, phone numbers, and hours of operation.

    Driver's License Forms

    Contact your local Oklahoma Department of Public Safety Office for the forms you'll need for driver's licensing, driver's license renewals, endorsements, and commercial driver's licensing.

    Driver's Tests

    Contact the Oklahoma Department of Public Safety Driver's License Testing Facilities. This link gives the addresses and phone numbers for the open testing stations. You must have an appointment to take a driving test at any of these facilities.

    Vehicle Registration Forms

    If you need forms for vehicle registration, license plates, tags, or anything else having to do with an automotive vehicle, motorcycle, motor scooter, or boat, contact your local tag agent.

    If you are not familiar with tag agents or with Oklahoma's vehicle registration process, don't let the tagging process confuse you. The tag agents are usually private parties, but their services are performed on behalf of and monies collected go to the Oklahoma Tax Commissioner.

    In the instance of registration needs, you can access most of the forms online from the Oklahoma Tax Commission website using the free Adobe Reader.

    The Tax Commission provides forms such as Application for Original Title, Vehicle Information Request, and U.S. Armed Forces Affidavit.
