    How to Replace a CDL

    Replacing a lost New York commercial driver license is easy. Simply apply to the Department of Motor Vehicles for a duplicate license and pay a $17.50 fee. You may do this online or by mail if your address hasn't changed, or you may apply in person.

    By Mail

    To apply by mail, download Form MV-44 and Form MV-44.1 (instructions for filling out Form MV-44). Make copies of your proofs of identity (contrary to what it says on Form MV-44.1, the DMV says that photocopies are acceptable for mail-in transactions). Mail the form, the proofs of identity, and the $17.50 fee to:

    The DMV has provided further instructions on applying for a duplicate license by mail.

    Since you may be ticketed for driving without a license in your possession, and you may only be issued a temporary CDL at a local DMV office while waiting for your permanent replacement license to be sent to you (this could take four weeks or more), consider bypassing the online and mail options and applying for a duplicate license in person.

    If Your License was Stolen

    The DMV will replace free of charge licenses that were lost due to a crime as long as you file a police report, obtain form MV-78B from the police, and apply for a duplicate license in person with the form at a local DMV office.
