Forms

Most Motor Vehicle Commission forms are in a Portable Document Format (PDF). To read and print them, you must have Adobe Acrobat Reader software and use version 4.0 or higher in order to fully use the features offered on some of the forms. (Earlier versions may not work well with all of them.)

Using a PDF form allows you to enter the required information while it is displayed on your computer screen and then print the completed form for submission. However, the Acrobat Reader does not allow you to save the form as a file, so you must print the completed filled-in form on white paper before closing it.

MVC Forms

The forms page on the MVC website has a selection of commonly used forms, divided into three categories:

Drivers: Includes Change Of Address Online form and a License Correction Application.

Vehicles: Includes an Emission Repair Form and the Application for Certificate of Ownership.

Business/Commercial: Includes the New and Used Motor Vehicle Dealer application.
