Unrestricted License

If you answer on your driver license application or license renewal form that you suffer from a medical condition that may cause seizures, blackouts, or loss of consciousness, you must provide the date when this condition last occurred and have your doctor complete a medical evaluation form. The Motor Vehicle Division (MVD) will then decide whether to issue you a license based on your doctor's report.
