Replacing a Lost CDL in Missouri

Apply for a replacement commercial driver's license with the Missouri Department of Revenue (DOR). You can apply in person if you're in state, or by mail if you're currently out of state. Alternatively, you MIGHT be able to renew your CDL IF it's due to expire in the next 6 months.

For your own protection, consider reporting your lost CDL to local and state authorities. Remember, it's illegal to operate a commercial truck or any commercial vehicle without your valid and properly endorsed CDL on you.

Order Duplicate CDL in Missouri

You can apply for a duplicate commercial driver's license in person at your local MO DOR driver license office.

Before you head to the office, have ready:

  • 1 document proving identity.
    • The DOR prefers photo proof such as an identification card from work, school, or the military; however, if you're unable to provide this the DOR will accept documents such as your Social Security card, one that shows your name and address, or one that shows your name and signature.
  • 1 document of your residential address.
    • If your residential address and mailing address differ, you also must provide proof of your mailing address.
  • The applicable duplicate fee.
    • $22.50 for a 3 year replacement.
    • $25 for a 6 year replacement.

NOTE: The MO DOR notes that additional documents MIGHT be required, depending on the situation. Refer to the state's list of acceptable documents and contact your driver license office for details.

MO Replacement CDL while Out-of-State

If you're currently out of state, you can order a duplicate CDL by mail.

  • Complete the Mail-In License Application (Form 4317).
    • Be sure to certify that your applicable commercial driver's license self-certification is in check.
    • You must provide your medical certificate or waiver if you choose “non-excepted interstate” OR “non-excepted intrastate.”
  • Provide all other documents listed on the application applicable to your situation.
  • Pay the applicable duplicate fee.
    • $22.50 for a 3 year replacement.
    • $25 for a 6 year replacement.
    • You can pay by personal check, money order, cashier's check, or traveler's check made payable to the “Missouri Department of Revenue.”
    • You must write your driver's license number on your form of payment.
  • Mail your application to the address listed on the form.

Lost CDL Processed as Renewal

If your lost CDL IS set to expire within the next 6 months, the MO DOR will process your duplicate request as a renewal.

Alternatively, if your lost CDL IS NOT set to expire within 6 months, you can request an early renewal. Contact your driver license office for details about this option.
