Change Your Name with the SSA

In some states, the DMV requires you to change your name on your Social Security card first, before updating your driver's license or car registration. Even though the Missouri DOR doesn't have this as an explicit requirement, it may be a good idea.

To change your name with the Social Security Administration (SSA), you'll need:

  • A completed Application for a Social Security Card (Form SS-5).
  • Documents proving your:
    • Identity (e.g. driver's license, passport, ID card).
    • U.S. citizenship, if applicable (e.g. U.S. birth certificate or passport).
    • Lawful U.S. presence, if applicable (e.g. work permit and I-94 print out).
    • Name change (e.g. court order, marriage certificate, divorce decree).

Send or take the above documents to your local SSA office.

The SSA will process your request and send you an update Social Security card within about 14 business days.

NOTE: Only your name will have changed on your new Social Security card. Your Social Security number (SSN) will remain the same.

MO Driver's License/ID Card Name Change

To change your name on your Missouri driver's license or ID card, you'll need to visit your local MO DOR office in person and apply for a duplicate card. You will need to:

  • Provide proof of your name change, such as your:
    • Marriage certificate.
    • Divorce decree.
    • Court order.
  • Have your photo taken.
  • Pay the duplicate driver’s license/ID card fee.

You may also be required to present proof of your:

  • Date of birth.
  • Place of birth.
  • Social Security number (SSN).
  • Missouri address.

NOTE: If your Missouri driver's license is due to expire within the next 6 months, you may be able to renew it instead of getting a replacement.

Car Registration & Title Name Change

To change the name on your vehicle registration, you'll need to first change your name on your certificate of title. To do this, visit your local Missouri DOR office in person and:

  • Submit a completed Application for Missouri Title and License (Form DOR-108).
  • Submit your current title certificate.
  • Provide proof of your name change (e.g. marriage certificate, divorce decree, court order).
  • Pay the $14.50 title fee.

You may also be required to present proof of your:

  • Date and place of birth.
  • Social Security number (SSN).
  • Residence address in Missouri.
