Where to Get Forms

Sorting through the paperwork is a major task for every dealer; that's why there are computer programs to help. All dealers, regardless of their class, must have an original title or other ownership papers for every vehicle or vehicle component in their possession.

Dealers can find an index of forms online where they can download many (but not all) of those forms that come from the Secretary of State's office. The forms are in PDF format, which you can view, save to your computer, print, and often even fill out on-screen using Adobe's free Reader software.

The forms you can't download from the Secretary of State's index of forms online, you must get elsewhere. Unfortunately, different forms are available from different sources. Fortunately, page 136 of the Dealer Manual lists which of the following sources you can contact to get each form:

  • Local Secretary of State Branch Office
  • Michigan Department of State:
    • Purchasing & Contracts Section
    • Licensing Section
    • Fax-on-Demand (see below)

  • Printer or business form company
  • Local law enforcement authorities

To order a catalog of the forms available from the Fax-on-Demand service, call (517) 335-4FAX (4329)
and enter the number of your fax machine when prompted. A catalog will be faxed to you.

Just as the forms come from different sources, so are they all returned to different places when they are completed. Instructions for both how to fill out the forms and where to send them should be given on each form.

And now, the list of forms needed for each type of license.

Class A (New Vehicle) Dealers

Depending on the nature of a dealer's activities, they may be required to have different records on hand. For Class A dealers, some or all of the following may be required:

  • Copies of processed RD-108s (Application for Title and Registration)
  • Police Book (hardcover or computerized) or washout system
  • Copies of front and back of conforming titles for odometer disclosure
  • Copies of separate odometer disclosure statements used with nonconforming titles, Manufacturer's Certificates of Origin (MCO), or new vehicle sales with electronic manufacturer's certificates of origin
  • Copies of purchase agreements
  • Titles or other ownership documents (TR-42, TR-52, etc.)
  • Copies of salvage disclosure statements
  • Copies of installment sales or lease contracts
  • Copies of Application for Original Michigan Salvage Title or Scrap Title forms (TR-12)
  • Copies of Salvage Vehicle Recertification Inspection forms (TR-13A, TR-13B)
  • Copies of temporary registrations and a separate log of temporary registrations
  • Copies of Scrap Vehicle Inventory forms (TR-9)
  • Copies of Major Component Parts Records (parts sales records)
  • Copies of auction receipts

Class B (Used Vehicle) Dealers

Forms needed by Class B dealers may include:

  • Title or other ownership documents (TR-42, TR-52, etc.)
  • Copies of processed RD-108s
  • Police Book (hardcover or computerized)
  • Copies of front and back of conforming titles for odometer disclosure
  • Copies of separate odometer disclosure statements used with nonconforming titles
  • Copies of salvage disclosure statements
  • Copies of purchase agreements
  • Copies of finance contracts
  • Copies of Application for Original Michigan Salvage Title or Scrap Title forms (TR-12)
  • Copies of Salvage Vehicle Recertification Inspection forms (TR-13A, TR-13B)

Class C and Class R Dealers

Forms needed by Class C and R dealers may include:

  • Copies of Application for Original Michigan Salvage Title or Scrap Title forms (TR-12)
  • Copies of Major Component Parts Records (parts sales records)

  • Copies of auction or salvage pool bills of sale

Class D and Class G Dealers

Forms needed by Class D and G dealers may include:

  • Police Book (hardcover or computerized)
  • Copies of front and back of conforming titles for odometer disclosure
  • Copies of separate odometer disclosure statements used with nonconforming titles
  • Copies of salvage disclosure statements
  • Copies of broker fee agreements
  • Copies of purchase agreements
  • Copies of RD-108s for retail sales
  • Copies of temporary registrations and a separate log of temporary registrations

Class E Dealers

Forms needed by Class E dealers may include:

  • Titles or other ownership documents (TR-42, TR-52, etc.)
  • Police Book (hardcover or computerized)
  • Copies of Scrap Vehicle Inventory forms (TR-9)
  • Copies of separate odometer disclosure statements used with nonconforming titles

Class F Dealers

Forms needed by Class F dealers may include:

  • Copies of Scrap Vehicle Inventory forms (TR-9)
  • Police Book (hardcover or computerized)

Class H Dealers
