In State
Go to your local exam station and ask the representative for the correct form to replace a lost CDL, and pay the appropriate fee. You will need to provide primary and secondary identification in order to receive a new CDL.
Out of State
If you're out of state, use the Driver's License Extension or Replacement form.
The fee is $16.

What Else to Do
Losing your CDL can be a frustrating experience. If you're out on the road, you must wait until you return to Kansas to have your CDL replaced. If you are stopped by a law enforcement officer, offer your manifest and driver's log and let the officer know immediately that you have lost your driver's license.
Also contact all the credit reporting companies to let them know to be on alert for suspicious activities.
If your license was in your wallet and the wallet was stolen, file a police report. The Kansas DMV recommends that you do, and that you keep a copy of the police report for your files. That way, you have additional proof to show any other law enforcement officer that may ask.
