Dealer Forms

There are a number of dealerships forms available for download from the Kansas Department of Revenue (DOR). You will not need all of them, but depending on the type of dealership you wish to open, you will need some of them.
The forms are available for download in PDF format, which means that you will need to install the free Adobe Reader software on your computer.
Where to Submit
Unless otherwise stipulated on the form, you may submit all motor vehicle dealer and salesperson related forms to:

Forms for New and Used Motor Vehicle Dealers
Here are links to the various forms needed if you are opening a motor vehicle dealership:
- Application for Location Change: If you plan to relocate the dealership or if you wish to open a branch office
- Application for Owner Transfer: Form to submit if the dealership is sold or otherwise transferred
- Application for a Vehicle Dealer License: New dealer basic application, used for new or used motor vehicle dealerships
- Auction Monthly Sales Reports: Form due each month before 30-day-permit orders can be released
- Basic Requirements to Become a Dealer: Checklist of the basic requirements that must be met before you will be granted a dealer's license
- Change of Record Form: Form used to change the names of records of the company owners or corporate partners
- Display Show Application/Extension: Used to extend the end-date at a motor vehicle display show at a local shopping mall, trade show, fair or other large, heavily attended event
- Franchise Certification: Form to submit when you have finalized your purchase of a franchise operation for a new motor vehicle dealership
- Full Use Plate Inventory: Used to track the dealer plates used by your company and to surrender any dealer plates not in use.
- Guarantee of Compliance with the Vehicle Dealer and Manufacturers Licensing Act Certificate of Deposit: Surety bond guarantee form
- New and Used Dealer Monthly Sales Report: Used to track the monthly inventory of new and used motor vehicle dealerships
- Personal Property Tax Certification: Required with the application for a new motor vehicle dealer's license and must be signed by the county treasurer in the city where the dealership will be located
- Replacement Plate and Additional Plate Request Form: Application to replace lost, stolen or destroyed dealer plates
- ST-8 Booklet Order Form: Used to order additional state sales tax booklets
- 30-Day Temporary Permit Order: Used to order your bundles of temporary permits, which will be sent upon receipt of your inventory report
- Trade Show Application Dealer Participant: Form used to apply to show motor vehicles at a trade show type of event, with the stipulation that no motor vehicle sales are to be conducted
- Trade Show Application Sponsor: Application required to sponsor a motor vehicle trade show event; also stipulates no vehicle sales are to be conducted
- Vehicle Dealer Bond: Form used as proof of the required $30,00 surety bond deposit, must be signed by an officer of the depository financial institute
- Vehicle Dealer Complaint: Used to allow consumers to file a formal complaint against a motor vehicle dealership to the Kansas Department of Revenue, Division of Vehicles
Forms for and Regarding Motor Vehicle Salespeople
- License Transfer Application for Vehicle Salesperson, Factory Representative or Distributor Representative: Used when a motor vehicle salesperson or factory representative moves from one dealership to another
- Original Application/Examination for Salesperson, Factory Representative or Distributor Representative: Form used when a new sales representative is applying for a motor vehicle salesperson license
- Vehicle Salesperson Exam: Exam questions to be answered by new motor vehicle salespeople for initial licensing and license renewals
Forms for Used Car and Salvage Dealers
- Basic Requirements to Become a Dealer: Checklist of the basic requirements that must be met before you will be granted a dealer's license
- Change of Record Form: Form used to change the names of records of the company owners or corporate partners
- Application for a Vehicle Dealer License: Used when applying to open a new dealership, whether for new or used motor vehicles
- KDOT Certificate of Compliance: For Kansas Department of Transportation to certify that the salvage facility meets the KDOT salvage storage and dismantling requirements
- New and Used Dealer Monthly Sales Report: Used to track the monthly inventory of new and used motor vehicle dealerships
- Salvage Pool Registration Application: Used as a declaration of ownership of a salvage pool
- Salvage Dealer's Major Component Part Quarterly Report: Report required four times a year to help track the movement of motor vehicle major components
- ST-8 Booklet Order Form: Used to order additional state sales tax booklets
- 30-Day Temporary Permit Order: Used to order your bundles of temporary permits, which will be sent upon receipt of your inventory report
- Used or Salvage Sales Report: Must be submitted to the Dealership Bureau in order to receive 30-day certificates
