The state has many forms for dealers, and fortunately, many of them are just a mouseclick or two away.

You can view the listing of the forms available online, and then print the ones you'll need. You will need the free Adobe Reader to download, read and print PDF files.

Make sure to fill out the forms completely, enclose any necessary payments and then mail them to:

  • Bureau of Motor Vehicles
  • Dealer Division, Warehouse
  • 6400 East 30th St.
  • Indianapolis, IN 46219

Some of the forms available online include a general dealer affidavit, the dealer application, affidavit for current insurance, and the application for dealer vehicle inspection identification.
