Medical Considerations

Sight
- You must score at least 20/40, with or without glasses or contacts.
- You must have a field of vision of 70 in one eye.
- You must possess the ability to judge distances and distinguish standard traffic colors.
Hearing
There no restrictions for deaf or hearing-impaired drivers.
Seizures
You must be seizure free for six months, although there is some wiggle room on this. Hawaii's county governments (there is no statewide DMV office) treat each situation on a case-by-case basis. If you are denied a driver's license for medical reasons, the state allows you to appeal the decision.
You cannot own a commercial driver's license (CDL) nor drive a bus if you have a history of seizures.
Aging Drivers
Drivers 72 years old or older must renew their license every two years rather than every six years.
Reporting an Incompetent Driver
If you suspect a driver of being too old or unfit physically to drive you can, if possible, confront immediate family members. If this option is not possible, contact your local police department with your concerns. Try to have the vehicle's license plate number.
Medical Report Form
There are no online forms available. You can pick one up at any of the following locations:
