DMV Forms in Delaware
Many of the forms and manuals you need to do business with the Delaware Division of Motor Vehicles DMV can easily be found on the DMV website. The forms are well-organized; you will be able to locate and print your forms easily. If you are applying for your driver's license, look for driver services (and if you are registering a vehicle, find the section on vehicle services).
If you don't want to download and print the forms, visit any DMV Office for copies of the manuals and forms.
If you have changed your address or your name, you'll need the Change of Address Form. Use the Visual Status Form (Form MV-312) to report your eye screening results. If your driver's license has been suspended, you may need an Request for Administrative Hearing (Form 533).
Many other forms and documents are also available for download at the DMV Forms and Manuals page.
The DMV recommends you transfer your title at a DMV Office so they can help you complete the paperwork properly.
Home or the DMV?
Because the Delaware DMV has retained so much control over the driver licensing and vehicle registration processes, you will not find every form you need on the Internet. But don't view an actual visit to the DMV Office as an inconvenience because, chances are, when you leave the DMV, your business will be successfully and thoroughly complete.
