Download Reader Software
Most of the forms necessary to conduct California Department of Motor Vehicles transactions are available in PDF format. All you need is the free Acrobat Reader software from Adobe. You can also fill out forms online, print them, and then either drop them in the mail or take them to the DMV.
Commonly Used Forms
- Request for Driving Record (or motor vehicle registration history)
- Traffic Accident Report (SR 1)
- Bill of Sale (used motor vehicles)
- Vehicle/Vessel Transfer and Reassignment Form (Reg 262 is not available online because of its sensitive secure nature)
- Statement of Facts (Reg 256 to waive title tax for in-family transfers of vehicles)
- Application for Copy of Title (Reg 227)
- Application for Disabled Parking Placard (with sections for the disabled person's physician to complete)
- Affidavit of Non-Use (Reg 5090)
- Power of Attorney (Reg 260)
- Medical Examination Report (commercial drivers DOT physical)
- Report of Vision Examination (DL 62)
But be advised that not all forms (such as the common DL 44 or DL 44C that are the first step in applying for a new license or commercial drivers license) are available to download. The reason these forms, and some others, are not available online is that they are controlled documents; each are assigned a unique barcode. If you need one of these forms you can call DMV’s Automated Telephone Service at 1 (800) 777-0133 to have one mailed to you.

Special License Plates Forms
Using these online forms, you can now fill out an application for a personalized or specialized license plate on your computer. However, you must print and mail the form along with the required fees to your local DMV office.

If you've got plenty of time for reading, the DMV serves up an online version of the California Vehicle Code. There's also a listing of new California laws regarding driver licensing and motor vehicles.
Informational Brochures
To make finding information even easier, the DMV also has a series of "Fast Facts" and "How-To" brochures designed to answer basic questions about driver licensing, motor vehicle registration, and other topics.
